I have been using a Resmed Airsense 10 Auto Set for Her for about 18 mths - its a struggle on and off - but lately find when stirring/waking (not getting up or taking the mask off) I cant get back to sleep easily unless I hit the ramp button. Last night I noticed the pressure was 9.5 at this point - tried for over 10 minutes for it to lower but it didnt - I usually dont look at this - should the machine pressure go down when awake (have a feeling the tech told me it would) Is it ok just to hit the ramp button - I did this 4 times the other night - does this affect the therapy. I am also noticing a noise - windy sort - but if I adjust my head on the pillow it seems to go away - feel its coming through the head gear - I am using a Air Fit N301 mask. Does it ever get simple and without angst?

I am not sure what you mean about hitting the ramp button? I did not know there was one and I use essentially the same machine. If you want the ramp to start over the quickest way to do it would be to hold your breath (assuming you are using smart start) and turning the machine off. Then breath and it will start again. Or if you are not using SmartStart then just turn it on again.

On the mask you may be hearing the air flow through the tubes that supply the air down each side of the mask. This would likely get worse if the weight of your head is squashing the tube on the bottom side down. Moving your head should correct it.

I ramp it again by turning it off - button on the top and waiting a few seconds and it starts again (smart start I guess) - so the ramp time starts again - starts low - so I can go back to sleep and builds up to around the 10 mark which is my average. Yep figure its just the tubing but havent found it as loud as it is recently. All good just a little annoying

I think that is about all you can do. I had a demo F&P SleepStyle machine for a while. It had a feature that it would detect when you wake up and automatically restart the ramp. I guess it worked. However this machine has other issues and I would not (did not) buy it. Overall I think the AirSense 10 is the best on the market.

This is not an unusual occurrence especially in REM. Pressures can hike up during that time because your muscles paralyze during that stage and apnea can get worse. Patients very often feel that pressure rise and wake up. Hitting the ramp button will restart your beginning pressure as well as turning off the machine. To answer the question of the time for pressure to go down: these machines are “smart machines” so they are designed to go up in pressure within a few breaths. It will decrease the pressure at a slower rate as it is expecting another occurrence of apnea and will not have to come all the way back up. Hang in there, not every night will be the same. Just remember that when pressures go up there is a reason. Your body is calling for it because of obstruction or there is high leak.
