Went from Philips Dreamstation to new Resmed aircurve 10v auto, what setting?

I lost my insurance while paying for my dreamstation so I ended up buying a Resmed Aircure V10 auto. I changed the settings in the Resmed and now have no idea what they should be. Below is a copy from the sleep study with settings...but what do I change in the autocurve service menu.

Welcome to the forum.
Based on the Physician Recommendations to further titrate Mode: VAuto (If you do not know what this is please ask) EPAP Min: 4 IPAP Max: 20 PS: 4

This will start you with an IPAP of 8.

I am going to ask you to download Sleepyhead and post your charts after the first night. Please include these charts, Event Flags, Flow Rate, Flow Limits, Snores, Pressure (not Mask Pressure), and Leak Rate.

The purpose of the above settings is to simply determine the best settings for you, they are a replacement for the titration study. Expect your settings to change after the first night.

What was the model of your DreamStation machine? Was it a BiLevel or an Auto CPAP? From what I see in your sleep study report the recommendations was to get a basic CPAP set at a fixed 9 cm of pressure. An Auto CPAP can also be set to fixed CPAP mode for the same treatment.

I guess I am wondering why you bought a BiLevel machine? I have seen studies which have found that in about 60% of the cases when a patient was switched from a CPAP or APAP to a BiLevel the outcome actually became worse, not better. What I am getting to is the thought that if you were getting good results with the DreamStation in fixed CPAP or APAP, that it may be better to stick with that setup rather than jump into the pressure support mode of a BiLevel. I have seen a number of posters here that have used a BiLevel when all they needed was an APAP, and to get the best results they have to switch it back to APAP or CPAP mode. You can essentially put the AirCurve 10 VAuto machine in APAP mode by setting the pressure support at zero. Or you can also put it into fixed CPAP mode. See page 14 of the technical manual at the link below.

If you really need a BiLevel I will leave it to bonjour to give you advice on how to set it up, as he uses one. The caution is that it may not be what you really need.

I will add that I have no problems with moving back to APAP, or even CPAP type settings if that is what will give you more effective therapy. I have done so in the past. The settings above are for titrating and they are what is typically recommended by most sleep clinics to use when a titration study is not performed. These settings are not typical for treatment, at least not in my mind as both Phillips and ResMed machines respond better when closer to the pressures needed to eliminate events, with ResMed having more latitude in settings than Phillips. These settings are set to see how both you and the machine react to the various settings and conditions. Adjustments would start on night 2.

The reason I went with the Resmed bipap was my insurance went away 2 months after the sleep study and the dreamstation. I could have paid each month at 60 or 70 for an additional 10 months, but I found a resmed bipap with 380 hours for 250....read up that I could set it either way, so I bought it. I tried with bipap on and it worked great...then I went in and changed a few setting...and being old and stupid, didn't write them down....That's why I am asking for help and why a have the Resmed. I remember first night on resmed and I just slept better and felt better than I had with the dreamstation. Will give settings a try and let you know.....

That was kind of what I suspected. Probably the best way out of the situation you are in is to post one or two of your ResMed AirCurve Daily Detail screenshots from SleepyHead. It should give a good idea where you are now, and how it could be best set up. To use SleepyHead if you do not have it, you need a PC or Mac and a SD card reader. The general process is to take the SD card from your machine and let SleepyHead copy the data on it. SleepyHead can then display the detailed data so you can see how well it is working for you. The software is free. Some links:

You got a good buy on the ResMed AirCurve 10 VAuto. You can do pretty much everything with it that you could do with an AirSense 10 AutoSet. About the only thing I can think of that is missing is the AutoRamp feature, which is nice but not essential. And the ability to use EPR in ramp only is there, but only for the CPAP mode.
