Got my new machine

hi all here is an update from my last post, as stated in my last post i was thinking about getting a new machine well i got the airsense autoset s10. since i have been using a fixed pressure machine for a while, i decided to go auto everything is set to auto also use warm up mode when i was first diagnosed 2 years ago i had 56 events within an hour my old machine was reading just under 5 my new machine is reading 2.0 no centrals, first night slept well but still waking up. finding min pressure strange to get used to, as the pressure on this machine ramps up when i am asleep and feel when i am awake with the mask on i am gasping for air, but i have been told i need time get used to auto mode, does any of you guys have any advice thank you in advance.

I could never adapt to the ramp so I just turned it off but mine is an older machine.

I think my last few neurons are off on holiday so answers are hard to come by at this time of night but I have had very severe apnea from way before acquiring my first machine about 4 years ago.

I certainly hope your new machine delivers every benefit you need. :)

I had wondered from your previous link what it was that killed your previous machine?

I'm asking because I still use a Resmed 9 and it's not running well atm.

Good to hear you got a new machine. The AirSense 10 AutoSet is a good machine, but it needs some setup to maximize the comfort. First off, it sounds like your minimum pressure is too low if you are awake in the night and it feels like you are suffocating for air. The machine will go as low as 4 cm pressure if you do not set it higher. 4 cm can feel suffocating, especially compared to a fixed pressure machine. What pressure was your old machine set at? That will give some idea how high the minimum therapy pressure should be set.

Here is how I set this machine up for comfort. See the Comfort set up options on page 16 of this manual. To get into the Setup Menu, press and hold the lower Home key and the round set button at the same time for about 5 seconds. Use the Home key to get back to the regular menu when you are done.

  • Ramp Time: Auto (This will hold the machine at the start pressure until you go to sleep)
  • Start Pressure: 7 cm or more (I would not set it at less than 7 cm unless your minimum pressure has to be less than 7 cm. You can't set Start Pressure higher than your minimum pressure. I adjust this pressure even higher to give me a feeling that there is no mask on my face at all. This pressure should be high enough that breathing is very free and easy)
  • EPR: On
  • EPR Type: Ramp Only (I saw from an earlier post you had EPR turned off on your old machine. With the Ramp Only setting it will only be on before you go to sleep for maximum comfort. After you go to sleep it will turn off, like you had on your old machine.)
  • EPR Level: 3
  • Climate Control: Auto (providing you have the heated hose)
  • Tube Temperature: 27 C

A little further down on the setup menu under Options you will find this item:

  • Essentials: Plus (Setting it to Plus will give you more options on your User Menu, and a more detailed Sleep Report.)

That is about it for comfort. The key part for now is getting your minimum Therapy pressure right to make it comfortable. I see from an earlier post you have SleepyHead. Have you put the data in for you new machine? It will be very helpful in fully refining your machine settings after you gather a bit of experience with it. Any questions about SleepyHead, just ask.

thank you so much for all your help sorry i never answered you earlier just got home from work. my ramp is set on auto so i change ramp and then put auto back on am i rnderstanding you , forgive my ignorence

Joe, what was your set pressure on the old machine? That would give me a better idea on what to advise you for comfort settings. To make the changes you likely need will require going into the set up menu. Are you OK with that?

OK, that helps a lot. Here are my revised suggested settings. Here is a link to a video that shows you how to get into the Clinical Menu to make these settings. The video is very good with a few exceptions. He recommends to use EPR full time, and I can't agree with that. As suggested below I would set it at Ramp Only. He also skips over increasing the Ramp Start pressure which I also think is important.

ResMed AirSense 10 AutoSet Setup

  • Maximum Pressure: 20 cm (that is the default and based on your old machine settings probably OK for now)
  • Minimum Pressure: 10 cm (I suggest this based on your old machine at 15 in fixed mode. With some review in SleepyHead you will likely have to increase that more, but 10 should be OK for now)
  • Ramp Time: Auto (This will hold the machine at the start pressure until you go to sleep)
  • Start Pressure: 8 cm (You can adjust this pressure to what feels comfortable to you and lets you breathe very free and easy while you are awake. I would not go below 7 cm though.)
  • EPR: On
  • EPR Type: Ramp Only (With the Ramp Only setting it will only be on before you go to sleep for maximum comfort. After you go to sleep it will turn off and provide better therapy.)
  • EPR Level: 3
  • Climate Control: Auto (providing you have the heated hose)
  • Tube Temperature: 27 C

A little further down on the setup menu under Options you will find this item:

  • Essentials: Plus (Setting it to Plus will give you more options on your User Menu, and a more detailed Sleep Report.)

I make these suggestions based on you feeling comfortable adjusting the machine. If you do not, then for sure see a sleep technician and get them to make the changes for you.
