Significant Changes in Mask Leak Readings and AEI Events upon changing to new ResMed 10 Auto

I’'ve been using a CPAP for about 14 years now with very good results. I just updated my ResMed 10 Auto to a new ResMEd 10 Auto as the older one was now at 5 years. Prior to doing this my average Events ranged from 0.2 - 0.7 with a very occasional 1.0. Also, my Mask Fit using the N20 Mask was nearly ball park perfect with a range of 17/20 - 20/20 according to MyAir web site. I use the N20 Mask and Climate hose and a chin strap. I've done this for the past 5 years with no problems and good results. I also use a SoClean2.

Since Dec 19, I've been through 3 NEW ResMed 10s -- and my results have been significantly different. First the explanation for the multiple ResMEds. The first CPAP started making a whistling noise after a couple days. I took it back to the DME dealer and they told me there was indeed something wrong; and replaced it with a 2nd NEW ResMed. This ResMed displayed some strange sounds and when I took it back the DME representative told me after examining it that there was some kind of "vibration" going on and a problem with reading numbers. The 2nd was replaced with a 3rd "NEW" REsMEd 10 which out of the bag failed to power on. After replacing the power cord, it has worked fine for 3 days now.

My primary issue is I've not changed my N20 mask nor the chinstrap. The Nose Cushion is new, the Climate Hose is new, a new filter installed -- and despite all of this the last 6 nights according to MYAir the Events have been 0.8 - 1.1 with a high of 1.8 two nights ago. In 14 years, the 1.8 is the highest I've ever experienced. Additionally, my Mask Fit has dropped to a recorded level of 9/20, 12/20 and 13/20.

ALL of this has occurred since going to the new ResMEd. Any advise/suggestions appreciated SleepBoomerMustang 73

HI SBM 73:

I have no suggestions - but a great deal of envy . I have never obtained such a low score such as .02 or 1.1 . Best for me is 6.5 AhI . If you are down to 1.1 that indicates better than most people who are not designated as OSA .

How did you manage to get this exceptional AHI score . Was it evever as high as 5.0?

Your numbers are still good. Have any pressure settings changed? EPR same? Is your mask type set to nasal?

Thank you Sleep innovator and PIK for your replies. I've continued to work with my Sleep Therapy Rep at Norco. No pressures remain the same (variable up to 13 if needed as prescribed by CPAP Dr. EPR same. Mask is set to Nasal for both the ResMed10 Auto and online with MyAir for the readings. My Norco Sleep Therapy Rep and I are beginning to wonder if ResMed has updated the technology with their new machines? These increases in mask leak and AHI ALL started with the new REsMed 10 Auto machines. Indeed, MyAir is NOT much help-- despite it is THEIR software providing the readings. I went into Norco and my Sleep Therapist did an analysis using my current N20 Mask running the entire range of settings --- and could NOT find any leaks or any problems with how I had the N20 Mask configured. YES, I do wear a chinstrap with the N20 Nasal mask as well. We also tried out a couple full face masks --- not fond of those; but will retry one over the weekend to see if it impacts my scores (AHI, Mask Leak, etc)
Either (1) ResMed has indeed changed their software settings again (which they deny as I emailed them inquiring) or (2) the latest ResMed 10 Auto CPAPs over the past 5 years have far more sensitive technology than in the past OR (3) for whatever reason the NEW (Dec 2019) ResMed I currently have has some kind of electronic issue with the reporting. Driving me crazy -- last nights Mask Fit indicated a score of 9/20!! I did not sense, feel nor hear ANY air leak. My wife did not either. Event indicated 1.2 I totally recognize I am still very lucky with an event score of 1.2 --- but would like to feel like I got a good nights sleep again -- and as long as those mask leaks stay WAY above 24 --- don't believe that is going to happen. We are considering testing out the Phillips Dreamwear CPAP -- although I've always heard the ResMed 10 is the top of the line and provides the most feedback via MyAir. Open to any suggestions.

A thorough analysis of the data yourself using free software called OSCAR would be very revealing. It requires a PC or MAC, and an SD CARD reader. If you still have the SD card from your old A10 it can be saved in OSCAR and compared to what you are getting now. Another option is to take your SD card to your sleep doctor and have them look at it. MyAir only looks at summary data. All the detailed data is on the SD card.

I will check to see if the Old SD card was left in my previous ResMed 10 vice transferred over to the new one.

I downloaded what I thought was the most recent version of OSCAR, but it failed the Norton Anti Virus test when scanned. I'll try again. Available at: https://www.SleepFiles.com/OSCAR

Yes, I thought of taking both SD Cards (if I still have both) into my Sleep Doctor and asking for a comparison scan. I've also talked with my Norco Rep regarding the possibility of resurrecting my old ResMed and try a couple nights to determine a comparison of MyAir./ResMed sleep scores since the significant (to me) changes all started with the beginning use of the New ResMed(s).

Last night MyAir scores showd a Mask Fit of 12/20 (previous night 9/20); Events last night 0.8 (previous night 1.2) and Mask Leak displayed a 32 (previous night 37). Sleep time 8:29 on Jan 17 and 8:06 last night Jan 18.

I will see if I can get the Oscar software downloaded and be able to operate it safely without any potential virus issues.

QUICK IMPORTANT UPDATE FOR SLEEP INNOVATOR... JANUARY 19, 2020 3:02 PM PST I was able to download successfully WITHOUT any virus warnings the OSCAR Software program. I will let the group know the results when I have them. Maybe it will help someone else in the future.

One thing to check for is the amount of CA events compared to total AHI. Central apnea can be somewhat erratic.

It is an open source software that is the successor to SleepyHead, on which development was discontinued. I still use SleepyHead, but it is not available for download any longer. It requires a PC or Mac and a SD card reader, and that your CPAP is compatible with the software. Most ResMed and DreamStation machines are, except for the very basic fixed pressure CPAP machines. You can download it at the link below. It allows you to look at all the detailed data that is being captured on your SD card.

I guess everybody has just a accepted this as the new norm? Several years ago I would comment on the new and upcoming customer service reps and say it is scary to think that when I can't take care myself anymore these are the people who will take care of me. I had no idea how scary and how far that went. I didn't go to medical school so I shouldn't have to download OSCAR and read my own charts. Doctors, nurses (pretending to be doctors) and DME suppliers are being paid millions of dollars a day by Medicare and by me to read these numbers for me.

When I did my sleep study, I had a 70 on my AHI. I am now anywhere from 2.7 to 26.5 with a monthly average of 11.7. My doctor's response...it's lower than 70. Ya' think? So I can assume as long as I keep it below 70, I am doing good. Next comment of the year...he asked me what I think we should do. I really don't know, give me a chance to check with Dr. Google and I'll get back with you. I guess I just need to download OSCAR, read and learn. Sadly, my days of learning are gone.

Many people with new CPAP machines like the ResMed AirSense 10 and the MyAir app think that all their data is being uploaded and "someone" is monitoring their results. Yes, very basic summary data is being uploaded, and your provider can (should) be looking at it. However, I suspect in reality that is not all that common. The other fact of life is that the real detailed data is only being collected on your SD card, and if nobody backs it up onto a PC, and you have a card failure, or reformat, or as I have done, put it in the wrong CPAP machine, then that data is all gone.

My advice would be if your monthly average for AHI is 12, then you need some further investigation. It may be something as simple as the maximum pressure is too low, the minimum pressure is too low, or you are suffering from central apnea and may need a new machine, or an underlying cause of the apnea investigation.

OSCAR is one way to do it. The other is to bring your SD card into your provider and ask them to do a though analysis of it, and explain to you why your AHI is so high. The goal is to be under 5.
